North Central CCH works with regional partners to reduce health disparities and inequities, building a more connected system of care that works for people, rather than a system that makes people work.

How does the North Central Community Care Hub work?

North Central CCH brings together community-based organizations, care coordinators and funding partners to strengthen our region’s ability to provide holistic, culturally responsive support. By working together to create a stronger, more connected system, we can better address the health and social needs of the individuals and families living in our region.

North Central CCH is a collaboration between Thriving Together NCW, Action Health Partners, and Advance. These partners lead specialized programs in health home coordination, recovery coaching and care coordination. Through funding, training, and peer support, North Central CCH helps community-based organizations expand their impact and deliver high-quality person-centered care.

Our trained Community-Based Care Coordinators provide care coordination that is local, in-language, holistic, culturally appropriate and free of charge.

Community Based-Care Coordinators focus on connecting people to health and social services, provide referrals, and track progress over time. With consent, they also help align health care providers and service partners to improve outcomes and strengthen support systems.

Who does the North Central CCH serve?

North Central CCH serves everyone in Chelan, Douglas, Grant, and Okanogan counties — individuals and families, young and old — who are struggling and suffering.

If you have clients or community members who are experiencing the following, please refer them to the hub:

  • Isolation

  • Systemic inequities

  • Lack of access to resources due to geography, discrimination, or language barriers

  • Substance use (alcohol and/or other substances)

  • Involvement with the justice system

  • Serious physical or mental health condition(s)

How can I become a partner of North Central CCH?

Everyone has a role to play in creating a healthier, more equitable North Central region. Meeting the complex needs of residents who are struggling or suffering requires a strong, connected network of both contracted partners and referral partners—each playing a vital part in the system.

The North Central Community Care Hub could not exist without our contracted partners, who provide direct community-based care coordination.

This trusted workforce walks alongside individuals and families with one-on-one, tailored, and culturally responsive support.

They help people connect to essential resources such as housing, food assistance, childcare, health care, and job training. Each contracted partner contributes unique expertise and extends our collective reach across the region.

A broader network of referral partners strengthens our system by connecting people to the Hub when they cannot meet a need directly.

Likewise, our care coordinators regularly refer clients out to these partners to address specific areas of support.

Some referral partners take their commitment even further by joining our Social Care Network. These partners ensure their services are listed in the statewide WA211 resource directory and, with client consent, share back general outcomes of referrals—helping us “close the loop” and build a more coordinated system of care.
