By contracting with and building the capacity of diverse community-based organizations, our North Central Community Care Hub helps strengthen a culturally and linguistically diverse workforce rooted in the communities they serve – communities often experiencing the highest rates of health disparities.

Our trusted helpers provide one-on-one, tailored, and culturally relevant support to connect people to health and social services, provide referrals, and track progress over time. They look at the whole picture and serve as liaisons, advocates and navigators.

Each of our contracted partners is a member of our Social Care Network and brings a unique contribution, including specific skills and geographic reach.
